First thoughts that came to my mind are ammo and headspacing.

I would find it odd for a reliable manufacturer to send out something like that.

Good possibility a 300blk out round could of ended up in there,or it’s a 223 barrel and shot a 556 round

How can you load a magazine and not noticing 300 Blackout among the 5.56 rounds?

There was a gentleman at the range jn the bay next to me shooting a .40 cal glock. He complained about a malfunction and how his brass had "glock bulge". I picked up a piece as I was policing my own brass and saw he obviously shot a 9mm in a .40 chamber.

People just don't always pay attention.

I forgot to mention, it was obviously not his fault, someone at the store must have swapped out a round.

Yes or no.
Some of these KB's clear the barrel while others leave the bullet half way down the barrel.
It likely depends on the bullet weight/construction and the charge.
Subsonics are going to leave a bullet in the bore while light bullet supersonics run higher pressure and would be more likely to clear the bore.
