Paperwork needed for PPT through the mail?

So, if I'm reading correctly, we need to send the following documentation along with the barrel/ upper to an agreed upon FFL:
1. A copy of the seller's DL.
2. An invoice including the normal address info for both parties.
2.A. If you put a dollar amount other than $0.00, the transferring FFL will have to collect taxes.

Please let me know if any of this information is incorrect.

just ask the receiving FFL, they will all be different.
